IX.a. Conflict of Interest Declaration Statement
Description:
A Conflict of Interest Declaration that needs to be signed by every Board Member. You can print ahead of time and bring with you, signed. Or, if not able to print, we will have blank copies at the mtg that you can sign and turn in.
If you aren't going to be at the first mtg you can print, sign and email back to Erin Breakie so that we can still have it on file, even though you won't be able to be there on Thursday. |
